The Position:

The role of the HBC Team Leader is to provide leadership and coordination of a team of case managers who are supporting children placed in Home Base Care. In addition to this you will work in conjunction with the Program Manager to plan and implement responses in relation to sector and service developments. This role is primarily responsible for coordination of delivery of a high quality program that provides genuine and positive outcomes for clients and carers. This role will participate in the development and maintenance of quality partnerships with key agencies including the Victorian Government Department of Health and Human Services (DHHS) and other community services providers and monitor performance targets, outcomes, quality standards in service agreements, legislative and policy requirements, quality of care issues, reportable conduct and complaints.

To Be Successful in this Position You Will Need:

  • Tertiary qualification in Social Work or other relevant area
  • Extensive experience in child, youth and family welfare experience, with particular experience in out of home care
  • Demonstrated experience in developing and leading a positive and engaged team environment.
  • Sophisticated understanding of the legislative, policy and practice environment of out of home care
  • Ability to lead and facilitate a multi-disciplinary care team
  • Skills and experience in supervision of carers
  • Current Victorian (or equivalent) drivers license
  • Demonstrated skills in case management
  • Well developed organisational skills
  • Program development and review/evaluation experience
  • Understanding of, and capacity to act according to the principles and intent of the Children, Youth and Families Act 2005
