The Team 

Future Pathways is part of AV’s approach to the Department of Families, Fairness & Housing’s (DFFH) state-wide initiative to support Family Preservation & Reunification (FPR). The Future Pathways team complement a suite of family supports provided within the FPR model. 

The Future Pathways program is an innovative approach built on practice elements such as motivational interviewing, communication skills building and goal setting to respond to identified needs and achieve real and demonstrable outcomes. Working collaboratively, the team seek to address key intersectionality issues with families, children and young people (e.g. substance use, mental illness, low income, social isolation) as identified by each regional FPR Response. 

 

This Role 

The Future Pathways Practitioner will:  

  • Actively participate in the Communities of Practice Forums and Case Consultation Meetings.  

  • Participate in supervision, reflective practice and consultation activities.  

  • Demonstrate compliance with agency requirements through clear and concise documentation and data entry.  

  • Provide direct engagement and motivation using methods compatible with principles and practices of the specified model.  

  • Contributes to the performance monitoring and reporting requirements of the Future Pathways program.  

  • Fulfil program requirements regarding case recording, data collection, registration standards and other requirements such as the Rapid Response evaluation.  

  • Record and update outcomes and data management systems to meet DFFH and Anglicare Victoria’s guidelines and statutory requirements.  

 

Role Requirements  

To be successful in this position you will need:  

  • A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences at degree level with substantial experience; or associate diploma level with substantial experience in the relevant service stream, or less formal qualifications with specialised skills sufficient to perform at this level. (Refer to Position Description for overseas qualifications) 

  • The ability to demonstrate competency against the knowledge, skills and personal qualities outlined on page 2 of the Position Description – such as: 

  • working with high needs complex families  

  • using a child focused family-centred approach 

  • utilising culturally sensitive practice with marginalised and minority groups 

  • knowledge of child welfare services 

  • understanding theoretical approaches (i.e., motivational interviewing and social inclusion) 

  • knowledge of evidence-based interventions 

  • experience conducting risk and needs assessments 

  • working within a casework framework 

  • The ability to perform work outside of standard office hours to service the needs of clients 

  • A current Victorian driver’s license and Working with Children Check and be prepared to undergo Criminal History Check(s) prior to commencement
