The Care Hub is a new initiative that brings together a number of organisations to form an integrated team of professionals to support Children and Young People in the areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.

Your role:
You will be joining a collaborative, integrated team of staff helping support those entering care for the first time.
The Case Support Worker will provide high quality administrative support to program staff, children, young people and families as part of the Care Hub program. Core duties may include detailed recording of client meetings, establishing client files and data entry. This is alongside providing practical outreach support to children, young people and families in conjunction with the multidisciplinary team as required.
