The Care Hub
Care Hub provides early intensive support & intervention to young people entering care for the first time; to minimise time in care & maximise successful family reunification. The Care Hub brings together a number of organisations to form an integrated team of professionals to support children and young people in the areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.

The Role
You will provide quality interventions and services that focus on outcomes for families experiencing vulnerability and focus on the safety and wellbeing of children. Your primary objective is family reunification.
The aim of this role is to provide intensive outreach to families (including outside normal business hours) with a focus on reunification and the safety and wellbeing of children and young people within the family home. The role will also undertake regular risk and needs assessments and case management utilising evidence based frameworks and interventions.
