Your role: 

As a Senior Case Manager you will oversee the care team of specialist professionals in delivering case management for high risk high needs young people across our homes in the south east region of Melbourne.

The position objectives are:

  • Provide support to young people in the program, and lead the multi-disciplinary team to develop and implement trauma informed plans for each young person. 
  • The Senior Case Manager is to provide leadership and coordination of the care team for each young person.
  • Provide direct support to young people in the KEYS program.
  • Lead the multi-disciplinary team to develop and implement trauma informed plans. 

 

Qualification requirements:

Due to the nature of the role and the duties and responsibilities attached to it you need to have:

  • Tertiary qualifications, preferably in social work / psychology, youth work or equivalent which includes diploma level qualifications.
  • Commitment to providing quality care and have experience in leading multi-disciplinary teams as a senior practitioner, trainer or mentor.

 

What you need to know:

  • A salary at SCHADS level 6 which equates to around $110,000 plus super at full time.
  • Flexible through the increments up to 6.3 (dependent upon experience)
  • Ongoing position, full time.
  • Site based at our office in Dandenong with outreach across the south east.
  • Pool cars available for your use.
  • A senior level role, supporting, developing and leading KEYS Youth Residential Support Workers.
