The Program:

The Navigator Program is an intensive, targeted, education support service where Case Managers work in collaboration with schools, young people, families, and external stakeholders to find ways to achieve educational goals.

Navigator provides assertive outreach and case management, supporting young people in government and non-government schools with extreme educational disengagement to re-engage. Many of these young people have complex barriers to educational engagement & have not responded to school-based engagement interventions. Working with young people, their families & support networks to address issues underlying disengagement & help them re-engage with their education & achieve their goals.

 

About the role:

We are currently looking for an experienced youth case manager; to work with our Navigator program - an innovative, research-informed program that assists disengaged learners to re-connect back into education and learning.

You will be working with young people (10 - 17yrs) who have disengaged from school across Western Melbourne. You will help young people identify their skills, interests and aspirations and alleviate barriers to their participation in education alongside their parents/carers. You will work collaboratively within a multi-disciplinary team, comprising of Team Leaders, Intake and Assessment Workers, Case Managers, a Groups Facilitator and other relevant services, bringing your own expertise in to the team.

 

The skills you will use day to day:

  • Specialty experience case managing young people with disabilities (intellectual, physical, learning and neurodivergence) and navigating service supports available for this cohort would be beneficial for this position.
  • Extensive case management experience, drawing from a variety of appropriate frameworks and theoretical approaches, working with children, families and/or significant others in relation to school engagement matters.
  • Successfully build rapport with young people, who present with a range of complex challenges, and to work towards client goals together achieving successful outcomes.
  • Excellent written and oral communication, negotiation and liaison skills and the ability to work with government and non-government organisations, community groups and clients’ families/networks.
  • Knowledge of school environments and educational policy

 

 What you need to know:

  • A salary at SCHADS level 6 which equates to around $110,000 plus super at full time.
  • Flexible through the increments up to 6.3 (dependent upon experience)
  • Fixed term covering parental leave to April 2027, full time.
  • Site based at our office in Yarraville with outreach across the region.
