We are seeking an experienced and passionate leader to oversee our Home Based Care and Targeted Care Packages (TCP) programs. This position provides leadership, operational oversight and practice guidance to a multidisciplinary team committed to achieving positive outcomes for children, young people, carers and families.
As Team Leader, you will champion high-quality, trauma-informed practice, support and develop staff, oversee program performance and drive continuous improvement across both service areas. Working closely with carers, families, Child Protection, Aboriginal services and community partners, you will ensure services are responsive, client-centred and delivered in line with legislative, contractual and organisational requirements. The role plays a critical part in creating safe, stable and nurturing environments that support children and young people to thrive. [
Home Based Care supports children and young people who are unable to safely live with their families by providing stable, nurturing and family-based care arrangements. The program includes foster care, kinship care and permanent care services and works collaboratively with carers, families and community partners to support children's safety, wellbeing, development and connection to family, culture and community. Home Based Care focuses on strengthening placements, supporting carers and working towards the best possible outcomes for children and young people.
Targeted Care Packages provide intensive and flexible support for children and young people in care who have experienced significant trauma and present with complex behavioural, emotional or developmental needs. The program delivers tailored therapeutic interventions and coordinated supports that help stabilise placements, reduce the risk of care breakdown and improve long-term outcomes. Working in partnership with carers, families, Child Protection and specialist services, TCP aims to build sustainable support networks that enable children and young people to remain safely connected to their community and care arrangements.
Position Requirements:
Required:
• Diploma of Community Services, Youth Services or similar
• Full Victorian Drivers Licence
Desirable:
• Territory qualification in social work, psychology or equivalent
• Shared Lives Training and Step by Step Training
