Your Role:
Intake is the central point to receive and manage referrals for the Western Region Home Based Care program.
The position will support our full time Intake worker and be responsible for managing our caregiver pool, matching/making placements, maintaining records and statistics and various reporting and compliance needs. The position will also support the co-ordination and management of emergency and respite placements for the region and carry some case management responsibilities as required. We anticipate an even split between intake support and case work functions, pending program need.
