Your role:
In this role, you will bring your experience supporting people impacted by long-term homelessness to our Lazarus Centre Breakfast Program at St Peter's Eastern Hill and St Mark's Community Centre in Fitzroy. Working within a busy and dynamic service environment, you will play a key role in providing specialist homelessness support alongside a small team of Support Workers and Volunteers who deliver meals, emergency relief and practical assistance to people experiencing primary and secondary homelessness.
You will be responsible for managing a complex case load and delivering client-centred, strengths-based case management to people experiencing significant barriers to housing, health and wellbeing. This is a highly autonomous role requiring a self-motivated practitioner who can confidently identify and engage people who may benefit from ongoing support, build trusting relationships, and support clients to actively participate in case management.
Drawing on your initiative, creativity and professional judgement, you will proactively identify opportunities for intervention, coordinate support responses, advocate across service systems, and work collaboratively with internal and external stakeholders to achieve positive outcomes. You will thrive in a fast-paced environment, demonstrate resilience when working with complex presentations, and be committed to providing flexible, person-centred support that meets people where they are in their journey.
