Your role:
You are going to provide case management and case contracting support through assertive outreach whilst consulting with a range of stakeholders to help young people meet their goals.
The position exists to provide a specialist case management service to young people who are considered to be at risk and residing in family based settings, independently or in home-based care placements. Managing a caseload of generally disengaged, complex, high-risk youth involved with Child Protection.
The service aims to strengthen the resilience of young people who are subject to protective involvement by providing a specialised, outreach based case management service. The young people who access the service are often experiencing multiple complicating factors such as:
