The Care Hub is a new initiative that brings together a number of organisations to form an integrated team of professionals to support Children and Young People in the areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.

Your role:
You will be joining a collaborative, integrated team of staff helping support those entering care for the first time.
The Family Services Practitioner within the Care Hub is to undertake assessment, coordinate the care plan including the placement plan and be the one point of contact for the child/ren and young people during their time in the hub. The role will coordinate the multidisciplinary team in coordinating assessment, planning and support children and young people in the hub.
