Your role:

Anglicare Victoria is the leading provider of out of home care services in the state, providing multiple programs for children and young people who are temporarily unable to live at home.

This leadership position sits within the broader out of home care program and is responsible for the Intensive Case Management Team and the Targeted Care Packages Team. Working with high risk young people who reside in OoHC placements, Parental Care and Independent Living.

Targeted Care Packages are an individualised model of support with funding tailored to the social and developmental needs of children, young people and their families who are subject to statutory orders. TCP’s are a creative and innovative support option aimed at transitioning children and young people out of or prevent them from entering residential care.

Intensive Case Management program is part of the Department of Families Fairness and Housing (DFFH) response to addressing the needs of high-risk adolescents up to 18 years of age who are subject to a statutory order. The program is outcome focused and draws from a strengths based/client centred approach to provide a high level of case management and case contracting support with an overall focus on safety, stability and wellbeing.

 

Make Your Mark:

  • Co-ordinate the delivery of a high quality program that provides genuine and positive outcomes for clients.
  • Lead and manage the delivery of a high quality, flexible and alternative care service.
  • Lead and participate in the management of all support staffing across the Intensive Case Management and TCP programs. 
  • Work in conjunction with the Program Manager to plan and implement responses in relation to sector and service developments.
  • Ensure appropriate processes are in place to enable continuous improvement of services and to promote best practice and ongoing quality improvement of the program.
  • Ensure the program is operated in accordance with service and quality standards, program targets, DFFH service agreements, organisational policies and accepted standards of practice.
  • Provide ongoing support, supervision, performance management, learning and development of team members in this program.

 

Make it work:

  • With excellent understanding of the child protection, placement and support services and family service system, including legislative and policy framework.
  • Excellent knowledge of, and experience in the application of relevant theoretical approaches that underpin casework practice (including theories of child attachment, development and abuse) to vulnerable children, young people and families.
  • Well-developed skills and experience in delivering programs and services within required timeframes and budget parameters.
  • Strong leadership skills with the maturity to guide and listen to people.
